We want to help you be as healthy as possible.

Our Disease Management programs can help you take care of chronic sickness.  A chronic sickness is when an illness does not go away or lasts a long time. Some examples are asthma and diabetes. When you are sick with a chronic illness, health coaches will call you and mail you materials to help you take care of your condition.

We offer disease management programs to our members who have: 

  • Asthma
  • Coronary artery disease (blocked arteries of the heart)
  • COPD (disease that makes it hard to breathe)
  • Diabetes
  • Heart failure

If you have seen a doctor for one of these illnesses, you may be signed up for the disease management program. If you aren’t signed up, you can join by calling 1-800-579-5755. If you are in the program, but don’t want to be, call the same phone number to get out of the program. 

 

Care Management Programs

Our Complex Case Management and Disease Management programs are designed to improve quality of life for affected members, to ensure they receive adequate and appropriate care, and to help manage their care cost-effectively. Use this form to refer a member to one of our case management programs.

Our programs combine the technologies used by leading medical authorities with the expertise of specifically trainednurse case managers who work directly with patients and their doctors. The programs promote quality of care to reducethe likelihood of extended, more costly health care.Areas we focus on for Complex Case Management are:

Diabetes
High-risk Pregnancy
Oncology
Respiratory
Transplant
Trauma
